Located in Philadelphia, Pennsylvania: St. Bartholomew’s LIFE Center followed the national PACE (Program of All-Inclusive Care for the Elderly) model, which provides robust, coordinated care and services in the community to enable older adults who need nursing home-level care to safely remain at home, maintain their health, and preserve their independence.

The project improves quality of life and health for older adults and reduces costs by avoiding unnecessary placement in costly settings including nursing homes or other care facilities.
