SUMMARY:Building the Data Backbone: Integrating Housing\, Health\, & Behavioral Health Data
DESCRIPTION:As health and housing systems become increasingly connected\, communities need secure\, practical ways to turn fragmented information into coordinated action. Join leading experts and practitioners for a discussion of how HMIS\, healthcare\, hospitals\, behavioral health\, Medicaid\, and Social Determinants of Health (SDOH) platforms can work together to improve care coordination and outcomes for people experiencing homelessness. Panelists will explore the opportunities and challenges of using linked data for research\, case management\, and real-time care coordination\, including privacy\, governance\, interoperability\, financing\, and trust across sectors. The conversation will highlight emerging approaches to building sustainable partnerships\, supporting closed-loop referrals and timely alerts\, and expanding access while protecting the people these systems serve. \n  \nPanelists\nDennis Culhane\nProfessor of Social Policy\, Founder and Faculty Director of Actionable Intelligence for Social Policy\, University of Pennsylvania  \nDennis Culhane’s primary expertise is in the field of homelessness and assisted housing. His early research helped to establish a field in the longitudinal analysis of homelessness based on the linkage of administrative data\, including establishment of a typology of homelessness that continues to guide homelessness policies and interventions globally.  He also was an innovator in the understanding of the cost of homelessness and the offsets associated with housing placement. He was an advisor to the Bush administration from 2003-2008 when he helped to design the Chronic Homeless Initiative.  From July 2009 – June 2018 he served as Director of Research at the National Center on Homelessness among Veterans at the U.S. Department of Veterans Affairs.  \n  \nShirelle King\nDirector of Healthcare Transformation and Social Care Integration\, Georgia Health Information Network (GaHIN)  \nShirelle King is Director of Healthcare Transformation and Social Care Integration at the Georgia Health Information Network (GaHIN)\, where she leads GeorgiaUnify\, the state’s Community Information Exchange. Her work focuses on bridging healthcare\, behavioral health\, housing\, and community-based services through statewide data exchange and interoperability. She has led initiatives connecting Georgia’s HIE with HMIS\, behavioral health providers\, health systems\, and community organizations to support whole-person care and closed-loop coordination. Shirelle is a nationally recognized speaker on health and social care integration\, and her work has received recognition from Civitas Networks for Health and InterSystems.  \n  \nEd Stellon\nChief External Affairs Officer\, Illinois Public Health Institute  \nEd Stellon serves as Chief External Affairs Officer for the Illinois Public Health Institute where he works to build integrated systems of care that improve health outcomes by addressing the social determinants of health. Ed spent most of his career at Heartland Alliance Health where he eventually served as Executive Director. That work allowed him to develop expertise and relationships in multiple human service sectors that have an impact on the lives of people experiencing homelessness. Ed has also worked with local\, state\, and federal government leaders to craft policies that improve access to care as well as effective services and supports for highly vulnerable people.
